Practice Based Commissioned (PBC) Services are enhanced NHS services that can take place within a community setting rather than patients having to travel to hospital. These services include:
They provide care for patients with conditions that can be safely managed within the community, such as a GP practice. This is part of the CCG’s commitment to delivering care closer to home.
All services are fully accredited and are commissioned by the CCG who monitor the services to ensure they are of a high quality.
Patients will be seen by someone who has specialist knowledge of their condition, this could be a GP or nurse who specialises in the area or perhaps a hospital consultant, but in a community clinic.
Although patients are being seen within the community rather than travelling to hospital, there are strong links between the practice based services and hospitals if patients require further follow-on treatment.
